It is the place where I chose to invest my dream in – the dream of a 22-year-old girl.</p> <p>My parents gave me a modest height of 156 cm, I dreamt of one day getting my height increased. About 4 years ago I happened to find a program on VTV2 about limb lengthening surgery in the US. The cost for the surgery was not cheap at all. I was very excited, every day I motivated myself to try to earn enough money to fulfill my dream.</p> <p>Last March, I read an article about the record of the longest length increased, which was 10cm, in Vietnam made by Assoc. Prof. Doan. Only then did I know that this type of surgery existed in Vietnam. I was excited to think about one day when I woke up with my height increased by 10cm, no one would be able to criticize me as the “pepper girl” anymore. With that thought in mind, I immediately called the doctor who performed the surgery to ask for advice. Unlike what I still thought, I would not be able to get my ideal height just after one surgery. In order to achieve that goal, I must go through at least 3 surgeries, facing a lot of pain and risks as well as going through perseverant treatment process and prolonged rehabilitation exercise. After the first surgery, I will have to stay in the hospital for 10 to 14 days, then I will be discharged to adjust from home every day. It will take abou 3-4 months to get the required length. That means I will have lie down and use a wheelchair during that long time before going to the second surgery. After that I will start exercising to be able to walk again soon. After about 18 months, the third surgery will be performed to remove the bone fixing device. When the bone is finally healed and becomes strong, I can return to my normal life and even play sports.</p> <p>At this point, perhaps most of you will advise me not to take risks, but for me, I must definitely follow this long-cherished dream of mine. If someone has done it, I can definitely do it as well.</p> <p>I was born and raised in the martial arts land of Binh Dinh. I am currently working for a company outside of Saigon. About a month ago, I flew to Hanoi to meet Assoc. Prof. Doan in person. After examining me, the doctor shook his head and decided not perform surgery on my legs because my bone structure was smaller than usual and could not take intramedullary nails for my legs to get lengthened. I cried a lot. Perhaps all hope are gone for me. Returning home, I was really depressed and didn’t want to do anything anymore. I locked myself in my room all day to cry. In my mind, there was only the thought of getting my legs lengthened.</p> <p>I burst into tears of joy when Assoc. Prof. Doan called to inform me that there were two small people like me who had surgery to have their legs lengthened without the need of intramedullary nails. He had changed the method and would be able to perform the surgery for me. I was so excited that I quickly booked a plane ticket to Hanoi.</p> <p>That was just the beginning, folks. I am actually not a strong girl. In order to realize my dream I also needed support. In the past, I have suggested this surgery to my mother many times, but she always rejected the idea and told me to stop thinking about it. She asked me if I was crazy. I was whole. Why would I want to break my legs and then fix them. I would lose a lot, including my years of life. I had never dared to talk about this with my father, because he was difficult. I also had great support from my boyfriend who encouraged me to do it. He promised that if there was an accident, he would take care of me (I doubt it (^_^)).</p> <p>I planned on hiding it from my parents until my plan was complete, I told them that I was off to Saigon for work. The night before I left, my mother could roughly guess what I was about to do. She told me, her eyes filled with tears, that if something bad happened to me, she could not live anymore. Knowing that I could not hide it anymore, I told my mother everything. My mother was really shocked. I quickly hugged her and burst into tears. “Please support me and let me fulfill my dream, mom. I have found a reputable hospital and a top specialist. You can rest assured that I will come back healthy to be with you again.” My mother also cried, “Staying there all by yourself, you have to be careful, okay? Me and your father will wait for your return.”</p> <p>To fulfill my dream, I had to accumulate wealth, found a caregiver to take care of me during the 3-4 months after the first surgery was performed when I would not be able to walk. I was certain that I would have to celebrate Tet alone in Hanoi and suffer the freezing cold of the winter in Northern Vietnam.</p> <p>My journey began…</p> <p>Stay tuned for the next part guys. This surgical method aims to serve the legitimate needs of many people, especially those of short stature which has caused many difficulties at work.</p> <p>Nguyen Thanh Hong Anh (23 years old, from Tuyen Quang) underwent leg lengthening surgery in the summer of 2015. Before getting her legs stretched at 108 Military Central Hospital, Hong Anh had to put up with gossips and hearsay. The female university student almost have to give up her dream of becoming a tour guide.</p> <p>In order to for the surgery to be successful, the patient have to endure a lot of physical pain.</p> <p>Upon hearing those gossips, the young female university student could “only bite her tounge” and rush to study to prove herself. As a result, Hong Anh was continuously awarded scholarships for being an outstanding student. However, in Hong Anh’s mind, she was always “obsessed” and suffered from self-deprecation about her height of 1.56m.</p> <p>Determined to improve her height, the tourism student learned about the leg lengthening method. After being explained by the doctors about the success rate as well as being told that the maximum length for her procedure would be 6cm because her body’s condition did not allow it to be extended any longer, Hong Anh decided to perform surgery.</p> <p>“After one summer at the hospital with strict exercise regimens, I’ve done it. My height has gone up by 6 cm. When my parents found out about my decision, my father was very angry but my mother understood my desire and supported me.<br /> After getting my legs stretched, I had to learn how to walk step by step like a child, withstanding all the suffering and shedding many tears. But now , I have a job at an Vietnam-based US travel agency, and life after surgery is no longer disturbed, ” Hong Anh shared.</p> <p><strong>Height increase surgery is not dangerous</strong></p> <p>In fact, cases similar to of Hong Anh is not uncommon. There are even people who have near “ideal” height, but for some reason they still have the desire to lengthen their legs.</p> <p>At the Institute of Traumatology and Orthopaedics – 108 Military Central Hospital, at least 10 cases of leg lengthening to increase height were taken each year, in which the number of male patients accounts for more than that of female patients. According to the doctor, self-deprecation related to height has hugh impacts on life, which leads to the need for limb lengthening. Prof. Le Van Doan said that leg lengthening surgery does not affect life expectancy.</p> <p>Assoc. Prof., Ph.D. Le Van Doan, Director of the Institute of Traumatology and Orthopaedics, said that leg lengthening surgery is the only way to increase height for people who have reached the end of their development age.</p> <p>Leg lengthening only applies to people with short stature (female under 150cm, male under 160cm) or people suffering from deformities and injuries. This technique does not apply to cases where leg length discrepancy is not more than 3 cm. With a small discrepancy, it can be improved by simply adding a shoe sole which is much easier.</p> <p>Regarding the appropriate age to perform leg lengthening, this expert said that it should be between 20 and 30 years old because at that time, the surgeon can determine the perfect height. After the age of 35, the bones begin to age, so they are no longer suitable for prolonged surgery.</p> <p>Finally, speaking about the impacts of leg lengthening on life and life expectancy, Assoc. Prof. Doan said, limb lengthening does not in fact affect the life expectancy of the surgical patients like many might mistakenly believe. However, after the process, the soft tissues (tendons, muscles, nerves, blood vessels, and ligaments) cannot keep up with the new situation right away. Therefore, rehabilitation exercises are recommended for a while, the duration will depend on the natural disposition of the body and specific desired length. Although she has 3 children, she still wishes to be 5.6 cm tall. She sought leg lengthening surgery even though she had received disapproval from everyone.</p> <p>M. is only 1.47 cm tall. Her modest height always made this mother of three feel inferior. Mrs. M. had a stable job and a happy family, but when her children get older, she began to want to have something done for herself. The first thing this mother wanted was to get leg lengthening procedure for her legs.</p> <p>Mrs. M. and her husband went to the hospital to consult with the doctor. Although the doctor said that she needed to consider it because she was already over the age of 35, the age that was not recommended to have any intervention, Mrs. M. was still determined.</p> <p>Seeing his wife so determined, Mrs. M’s husband did not dare to dissuade her. To prepare for the leg lengthening osteotomy, Mrs. M. resigned from her job and found someone to take care of her family as well as her during her legs’ recovery time.</p> <p>It was very similar with the case of Tran N.H., 21, from Ho Chi Minh City. H. was an international student in Northern Europe. During his time living among big people, this male student always felt inferior because he was quite short (1.64m). Although he wore high-soled shoes, compared to his friends, H was still short. H. learned about leg lengthening surgery and decided to take a year off to return to Vietnam for surgery. H. came to us to express his wish.</p> <p>The surgery helped increase H’s height by 7cm which is what H and his family have always wished for.</p> <p>Assoc. Prof. Doan said that leg lengthening is everyone’s need, however patients will receive strict prescription from the doctor. In order to achieve good results, the doctor will provide advices to each patient.</p> <p>According to Assoc. Prof. Doan, in theory, in order to perform leg lengthening, you have to cut the bone, then use a tool to pull the two parts bone apart. When stretched, the two parts of the bone gradually move away from each other, forming a gap at first. The periosteum is the place where osteoblasts will regenerate and grow into new bone.</p> <p>However, depending on the location and age of each person, the regeneration rate will be fast or slow. The younger you are, the faster you can regenerate. For the restored bone to become as strong and big as healthy bones, from the time the stretching begins, the rate should be between 35 and 40 days for 1cm.</p> <p>The principle of limb lengthening, which is still applied today, was established from 1951, nearly 70 years ago. The principle is to perform osteotomy, then stretch it at a rate of 1mm per day.</p> <p>Before, doctors had to pierce 8 nails through the bone and install an external fixator to slowly stretch at a rate of 1mm per day. When stretching to reach the desired height, the patient had to wear the fixator for 7-10 months, even up to the next 12 months for the bone to solidify, before it can be removed.</p> <p>That means the patient had to wear the fixator during the whole procedure, so it was very cumbersome during daily activities, not to mention there were complications such as pin site infection, misalignment or rotation of the axis, and limb deformities during stretching.</p> <p>Currently, 108 Military Central Hospital has applied new innovation to the procedure. Before osteotomy is performed, the doctors will put a nail in the cavity of the bone marrow. They has also improved the fixator by using only 4 small nails pierced through the 2 ends of the bone.</p> <p>When the leg is stretched to full length, the fixator will be removed. The nail located in the cavity will hold the role of fixation while waiting for the bone cavity to solidify.</p> <p>Leg lengthening surgery is a very difficult surgery, so almost not everyone can do it. In Northern Vietnam, there has been only 108 leg lengthening surgeries. Surgery can also result in complications which can put a lot of weight on the doctors’ shoulders.</p> <p>If the doctor’s skills are not good, complications can occur. Complications during surgery can also occur such as improper osteotomy, broken bones, or vascular and nerve damage. Not to mention infectious complications.</p> <p>Osteomyelitis is also considered a failure when it happens to leg lengthening patients. They may also experience post-operative complications like when the stretching begins. These complications include slow bone healing, nonunion, weak bone cavity.</p> <p>Up to now, the hospital has performed for about 500 cases. No complications have been recorded and everyone is satisfied because they have improved their height. Prof., Ph.D. Le Van Doan, Director of the Institute of Traumatology and Orthopaedics, 108 Military Central Hospital can still remember vividly a female patient named Hang from Hanoi who was led by her sister who is a medical worker to the 108 Hospital to seek advice on leg lengthening.</p> <p>“The female college student that I encountered was as small as a piece of candy with a height of only 1.38m. Her weight was over 30kg and she was in her junior at an university in Hanoi”, Assoc. Prof. Doan recalled.</p> <p>Her inferiority complex about her height prevented her from socializing with her peers. This method was for people who were brave and persistent as it would take nearly one year to wear the fixators. With all personal activities being done from the bed, it would be very frustrated and uncomfortable.</p> <p>Without any hesitation, the young girl decided to take a year off from the university to start her leg lengthening journey. More than 300 days passed full of tears and pain. Finally, Hang burst with joy as her height has increased by 8cm.</p> <p>She returned to the university with a joyful attitude. Later on, she moved to Ho Chi Minh City to start her career and get married and now she is enjoying her life there.</p> <p>Dr Doan states that Hang is the shortest patient to receive leg lengthening surgery from him.</p> <p>Two other female patients who were both 1.53m tall and 37 years old also decided to get leg lengthening together though both have had 2 children already. At the moment, they have finished the treatment with perfect results, each getting their height increased by 7cm. Up to now, these are the 2 oldest cosmetic leg lengthening patients that come to the 108 Hospital.</p> <p>Recently, the Institute of Traumatology and Orthopaedics – 108 Military Central Hospital has received 2 special patients coming for leg lengthening. The first case is a Vietnamese American international male student, who is 18 years old this year. He was 1.60m tall, has got his legs stretched by 6cm, and will have the fixators removed next week.</p> <p>The other case is a girl of Vietnamese origin who is living and working in the UK. She has decided to take a year off from her work to return to Vietnam to undergo surgery to improve her height of 1.49m due to her inability to integrate into the work environment.</p> <p>Dr. Doan also cannot forget the case of a 25-year-old man from the Southern Vietnam who flew to Hanoi to apply for leg lengthening surgery. A long-term plan was even made by this man by accumulating wealth, renting a house, hiring a caregiver for nearly a year in Hanoi.</p> <p>After completing all the tests to be ready for surgery, Assoc. Prof. Doan asked about the person accompanying the young man to see if he was just a friend and not a relative. Soon after, the patient’s mother found out about her son’s intention and contacted the doctor to stop the surgery.</p> <p>Hearing the doctor’s announcement, the young man almost went crazy. Because he was only 162cm tall, when he first came to a foreign country, Minh became insecure about his height in comparison to his foreign classmates. Minh even had to look up to some Korean girls. Motivated to get limb lengthening, Minh read about the process, duration and cost for a limb lengthening procedure. Thanks to his proficiency in foreign languages, Minh was able to learn about different lengthening techniques around the world. Although there were a lot of medical innovations in China, Chinese laws had not permitted the pratice of limb lengthening. In European and American countries, a limb lengthening procedure would cost more than 100,000USD, which was a huge amount of money, beyond his family’s financial capability. He almost thought that he had to leave his dream behind until he had a job to save up the required money.</p> <p>But fortune smiled upon Minh. While he was doing his research online, he found a post from a Vietnamese Canadian patient who was working and living in Toronto. He had flown to Vietnam to undergo a limb lengthening procedure. From the post, Minh learnt about Assoc. Prof. Le Van Doan who has a lot of experience in orthopaedics and has published many researches on limb lengthening. After his research, Minh expressed his wish for improving his height to his parents. Although they were being supportive, both of them were concerned. They then made an appointment and came to the hospital to get information about this technique, especially the risks that could come from leg lengthening surgery.</p> <p>Right after his first year in college, on July 2017, Minh decided to suspend his studies to come back to Vietnam to fulfill his dream. After some examinations , Assoc. Prof. Doan adviced Minh to have a tibia lengthening and try to increase his height by 8cm. The risk of complications, especially Achilles tendon contracture, would be low. The procedure was successfully performed as planned. After 10 days, Minh was discharged to come back home and follow the doctor’s instruction. He had examination every 4 weeks. After the last examination at week 11, before the process was halted and the fixators were removed, with the determination and desire to reach a maximum height, Minh’s consistent execise helped his knees and Achilles tendonts avoid contracture. When he went to the doctor, Dr. Doan was quite surprised that Minh’s knee and ankle flexion nearly reached the normal level. For this reason, the doctor agreed and advised Minh to again increase his height by 2cm. On February 2018, Minh could walk on his own without the help a crutch. On August 2018, Minh returned to college to continue his education. His old friends could not recognize him with his new height of 172cm.</p> <p>When he was asked about his decision, Minh felt satisfied with his results. His surgical scars were small and are fading over time. People around him could not notice it if he did not tell them. Now his legs are healthy again. He can walk and run normally. Up to now, there have been many studies on the application of stem cells to restore the function of organs and body parts with very positive results.</p> <p>In the field of beauty care, stem cells are also researched and applied by many prestigious beauty centers around the world.</p> <p>Since 2008, the research team from the Institute of Traumatology and Orthopaedics, led by Dr Doan, has reported on the applications of stem cells from autologous blood to treat difficult-to-heal musculoskeletal injuries. Particulary, there have been applications in leg lengthening methods. The fixator is dedicated only for leg stretching. It was registered and licensed at the Intellectual Property Office of Viet Nam – Ministry of Science and Technology. The fixator is designed for simple operation, consisting of 3 pile rods with opposite threads. The stakes are connected with 2 semicircular rings at the 2 ends. Each semicircle holds 2 small nails pierced through the bone. At the center of the rods are the sides that form a hexagon. Therefore, it can be adjusted to stretch or squeeze easily when desired. Turning one round is to stretching or squeezing in 2mm, so turning 3 sides a day is equal to 1mm and turning 1 side is equal to 1/3mm. Prof. Le Van Doan stated that limb lengthening, particularly leg lengthening, is not a complex procedure. This technique has been developed for over a hundred years, originally created to treat patients with leg length discrepancy caused by sequelae from war injuries or labor, cases of osteomyelitis or the need to cut bone tumors; patients suffering from polio; etc.</p> <p>In Vietnam, since 1995 when polio was eliminated thanks to the expanded vaccination program, the number of cases of leg length discrepancy has also decreased significantly. However, the causes of trauma after accidents and osteoarthritis still cause many patients to suffer from this deformity. In particular, the need to undergo leg lengthening for aesthetic purposes has also increased significantly.</p> <p>At the Institute of Traumatology and Orthopaedics, 108 Military Central Hospital, each year Assoc. Prof. Doan and his colleagues perform dozens of stature lengthening procedures to increase height. Particularly, the number of male patients is greater than that of female patients. According to the doctor, self-deprecation related to height has hugh impacts on life, which leads to the need for limb lengthening. For men, this type of self-depracation is more clearly felt.</p> <p>Assoc. Prof., Dr. Le Van Doan said, in theory, the leg length can be stretched as much as possible, depending on the needs of the patient. But experts always advise them of the right length that is proportional to the body as well as ensure minimal complications.</p> <p>Regarding the method of implementation, in the past, we used the osteotomy method, piercing 8 nails through the bone and installing an external fixator to slowly stretch at a rate of 1mm per day. Thus, if you wanted to extend 7 cm, it would take 70 days. When it reached 7 cm, it ws still necessary to wear the frame for the next 7 months to make the bone strong, then you could remove the fixator. So the patient had to wear the fixator for a long time up to 10 months, which was very cumbersome in daily life. Now, thanks to the application of new advancement, before performing osteotomy, doctors will put a nail in the bone marrow canal, using only 4 small nails through the two ends of the bone. When stretched to full length, the fixator will be removed. The nail located in the canal will hold the role of fixation while waiting for the cavity to solidify. Thus, with this new method, external fixation duration has been shortened, to only ¼ of its original duration, the early removal of the fixator help the patient to quickly return to daily work and life. The scars are also small and their number is reduce greatly.</p> <p><strong>Who can undergo leg lengthening?</strong></p> <p>According to Assoc. Prof. Le Van Doan, indications for leg lengthening are only for people of short stature (female under 150cm, male under 160cm) or people suffering from deformities and injuries. This technique does not apply to cases where leg length discrepancy is not more than 3 cm. With a small discrepancy, it can be improved by simply adding a shoe sole which is much easier.</p> <p>Regarding the appropriate age to perform leg lengthening, this expert said that it should be between 20 and 30 years old because at that time, the surgeon can determine the perfect height. After the age of 35, the bones begin to age, so they are no longer suitable for prolonged surgery.</p> <p><strong>After the lengthening, will the leg get weak?</strong></p> <p>Regarding this relatively common concern, Assoc. Prof. Doan said, limb lengthening does not in fact affect the life expectancy of the surgical patients like many might mistakenly believe. However, after the process, the soft tissues (tendons, muscles, nerves, blood vessels, and ligaments) cannot keep up with the new situation right away. Therefore, rehabilitation exercises are recommended for a while, the duration will depend on the natural disposition of the body and specific desired length. Short stature is only a relative concept when talking about height.</p> <p>We can take a person’s height that is less than the average height (by gender and age group) minus 2 times the standard deviation of the average height to evaluate if that person has short stature. According to survey data published in 2003 by the Vietnamese Ministry of Health, at the age of 20-24, the height of men is 163.72 ± 4.67 cm and that of women is 153 ± 4.32 cm; at the age of 25-29, the height of male is 163.71 ± 4.77 cm and that of female is 152.73 ± 4.12 cm.</p> <p><img loading="lazy" class="aligncenter size-full wp-image-146" src="https://cdn.drdoan.vn/wp-content/uploads/2021/07/190115-3.jpg" sizes="(max-width: 1007px) 100vw, 1007px" srcset="/wp-content/uploads/2021/07/190115-3.jpg 1007w, /wp-content/uploads/2021/07/1_190115-3-300x170.jpg 300w, /wp-content/uploads/2021/07/1_190115-3-768x434.jpg 768w" alt="" width="1007" height="569" /></p> <p>Short stature is a result of many affecting factor including genetic makeup; gonadal, pituitary and thyroid hormones; rickets or a mixture of various forms. In addition to the above factors, short people can be divided into two main groups:</p> <ul> <li>Dwarf: These are people with short stature caused by dysplasia. These people are very short compared to the average person and have disproportionate body parts and deformed bones. Patients require a plan for leg, thigh, and arm lengthening from an early age, with multiple stages of treatment.</li> <li>People with short stature that is not caused by dysplasia sản (constitutional short stature; low normal stature): These people are not too short compared to the average person and dó not have disproportionate body parts.</li> </ul> <p>A beautiful body must not only possess the ideal height, but also need to have proportional body parts, especially the length of the legs compared to the torso. The Vallois and Skelie indexes have been used in anthropometry to assess the proportion between the limbs and the torso.</p> <p>Vallois index (torso index) = Sitting height x 100/Standing height.</p> <p>Based on the Vallois index, we can divide people into the following groups:</p> <ul> <li>Short torso if the Vallois index is equal or less than 50.9.</li> <li>Average torso if the Vallois index is between 51 and 52.9.</li> <li>Long torso if the Vallois index is equal or greater than 53.</li> </ul> <p>Skelie index (leg index) = Lower limb length x 100/Sitting height</p> <p>Lower limb length = Standing height – Sitting height</p> <p>Based on the Skelie index, we can divide people into the following groups:</p> <ul> <li>Short legs if the Skelie index is equal or less than 84.9. In particular, people have very short legs if the Skelie index is less then 74.9; people have relative short legs if the Skelie index is between 75 and 79.9; people have slightly short legs if the Skelie index is between 80 and 84.9.</li> <li>Average legs if the Skelie index between 85 and 89.9.</li> <li>Long legs if the Skelie index is greater than 90. In particular, people have slightly long legs if the Skelie index is between 90.1 and 94.9; people have relative long legs if the Skelie index is between 95 and 99.9; people have very long legs if the Skelie index is greater than 100.</li> </ul> <p><strong>Leg lengthening characteristics for people with short stature:</strong></p> <p>It is necessary to carefully study the reproductive history, the growth in stature during adolescence, the time of puberty, and the hormones history. People with early puberty, hormone deficiency, childhood illnesses, genetic diseases, etc. have a high risk of short stature. Family history also needs to be carefully studied, including the parents and siblings. Psychological status also needs to be assessed. Is the person’s psychology affected or is he having an inferiority complex due to his short stature? How are the frequency of effects and general psychological status? From there, the surgeon can select patients with good indications to perform leg lengthening. These are people who have often inferiority complex due to their short stature, but in other aspects of their lives they are still successful, happy, active and most importantly these patients have realistic expectations of what leg lengthening surgery can do for them that are not just merely associated with cosmetic benefits. Leg lengthening surgery is only for those who feel the need to do it and are mentally healthy.</p> <p>At the same time, it is necessary to have examinations to assess the height and range of motion of the joints. Is there any damage to the nerve and blood vessels? Is there any joint deformity? Is the limb deformed? What are the measurements of the legs and does leg length discrepancy exist? X-ray scan of the lower limbs should be taken to evaluate the limb axis, tibial and femur length. Diameters of the femur and tibia where they are narrowest on straight and inclined film should be measured.</p> <p><strong>The age suitable for leg lengthening:</strong></p> <p>Currently, there is no the upper age limit for leg lengthening. Leg lengthening can be performed after the person stop growing in height, when the X-ray scan of the leg shows no cartilage which is usually over the age of 18. There has been a case of leg lengthening in the world in which the patient was 47 years old at the time.</p> <p><strong>The position and the increased length:</strong></p> <p>The position of leg lengthening can be at the shank, thigh or both. However, most doctors would choose the two shanks to perform leg lengthening. The reason behind this decision is to avoid disproportionate body and reduce treatment time and cost in comparison with those of the procedure on two thighs.</p> <p>Assoc.
